• Child’s physician’s name, address and phone number.
• Child’s dentist’s name, address and phone number.
• Name, address and phone number for two local emergency contacts.
• Medical insurance provider and group number.
• Dental insurance provider and group number.
• Date of your child’s last DTP vaccination.
• Credit card to pay the registration fee.
